血浆己糖神经酰胺(d18:1/22:1)与 2 型糖尿病发病的关系

摘要:目的:通过靶向代谢组学研究中国成年居民血浆己糖神经酰胺(hexosylceramide, hexcer)(d18:1/22:1)与2型糖尿病(type 2 diabetes mellitus, t2dm)发病的关联。方法:在中国苏州市工业园区社区人群中开展的巢式病例对照研究。测量了234例t2dm病例和468例对照者的血浆hexcer(d18:1/22:1)。方法:采用条件logistic回归模型探究血浆hexcer(d18:1/22:1)与t2dm发病之间的关系,计算比值比(odds ratio, or)及95% 置信区间(confidence interval, ci),并拟合限制性立方样条(restricted cubic spline, rcs)模型评估剂量-反应关系。结果:多因素调整后,hexcer(d18:1/22:1)位于第四分位组的人群t2dm的风险显著增加,or(95% ci)为1.81 (1.12, 2.92),与hexcer(d18:1/22:1)正常组相比,hexcer(d18:1/22:1)升高组发生t2dm的风险显著增加(or 1.62,95% ci:1.11, 2.36);此外,血浆hexcer(d18:1/22:1)的水平每增加一个对数转换标准差,t2dm的发病风险增加28%。rcs模型分析显示,血浆hexcer(d18:1/22:1)与t2dm的发病风险存在线性剂量反应关系(p线性<0.05)。结论:研究揭示了中国成年人群中hexcer(d18:1/22:1)与t2dm之间的关联性,高水平hexcer(d18:1/22:1)可增加t2dm的发病风险,且存在线性剂量-反应关系,hexcer(d18:1/22:1)可能是t2dm发病的潜在生物标志物。

abstract:objective: to investigate the association between plasma hexosylceramide (hexcer) (d18:1/22:1) and the development of type 2 diabetes mellitus (t2dm) in chinese adult residents by targeted metabolomics. methods: this was a nested case-control study conducted in a community-based population in the suzhou industrial park, china. plasma hexcer (d18:1/22:1) was measured in 234 t2dm cases and 468 controls. conditional logistic regression models were used to explore the association between plasma hexcer (d18:1/22:1) and the onset of t2dm, and odds ratio (or) and 95% confidence interval (ci) were calculated, and restricted cubic spline (rcs) model was fitted to assess the dose-response relationship. results: after multivariate adjustment, the risk of t2dm in the fourth quartile of hexcer(d18:1/22:1) was significantly increased, with or (95%ci) of 1.81 (1.12, 2.92). compared with the normal hexcer(d18:1/22:1) group, the risk of t2dm in the elevated hexcer(d18:1/22:1) group was significantly increased (or 1.62, 95%ci :1.11, 2.36). in addition, the risk of t2dm increased by 28% for each increase in the level of plasma hexcer(d18:1/22:1) by log-transformed standard deviation. rcs model analysis showed that there was a linear dose-response relationship between plasma hexcer(d18:1/22:1) and the risk of t2dm (p linear <0.05).conclusion: the study revealed an association between hexcer(d18:1/22:1) and t2dm in the chinese adult population, high levels of hexcer(d18:1/22:1) increased the risk of developing t2dm, and there was a linear dose-response relationship, and hexcer(d18:1/22:1) may be a potential biomarker of t2dm pathogenesis.
